Let’s Prevent Falls and
Celebrate Healthy Aging!
We would like to highlight and celebrate the resilience of adults 55 or better and showcase Healthy Aging and how impactful Community-Based Evidence-Based Health Promotion Programs can be. This year for Falls Prevention Awareness Week (September 23-27, 2024), we invite all healthy aging and fall prevention program leaders, participants, organizations, and coalitions to share their stories.
We invite you to submit photo and video testimonials to inspire others to explore healthy aging and to show gratitude for your community/program providers and fellow participants!
How it Works
The region or coalition with the highest participation (photo or video) will be prominently featured throughout the year and honored as the 2024 NC Falls Prevention Awareness Week (FPAW) Trailblazer. Individuals not connected to a coalition are more than welcome to submit entries, and let us know if you want to be connected to a regional coalition or even start one!
The winners will be recognized for their dedication to promoting healthier, empowered communities and preventing falls among adults. This accolade celebrates their commitment to fostering healthier lifestyles, and underscores the resilience and determination shown by program participants, leaders, and advocates alike.

2. Show how you (and your family and friends if you wish!) stay strong, healthy and physically active! ALL AGES welcome and encouraged! Parents/Guardians need to sign media consent for those under 18.

Submission Deadline: August 30, 2024
The required consent form is on the submission portal link - please have the individual in the photo/video complete & sign the consent form. Please try to keep written testimonials to ½ page - 1 page. Only submit one photo at a time and complete the submission form questions. If you are not associated with a region/coalition, you are still encouraged to submit a testimonial and we can connect with you!
If the individual is not submitting a photo/video on behalf of themselves, they can follow this direct link to the photo/video consent form or you can print the consent form from page 5. Note: If you are submitting a photo/video of yourself, you do not need an additional form.
These photos will be used and highlighted in the Standing Strong NC Blog and in the 2024 NC Falls Prevention Awareness Week Promo Materials.

If you have had a fall, what made you decide to join a community program and how have you benefited from it?
What attracted you or made you want to join when you heard about the program?
What happened during the first class? What made you want to stay and complete the whole program?
What did you like the best about the program? What is one key lesson or takeaway?
What are you doing differently as a result of taking the class? How has the class changed your life?
What do you think it was about [program name] that helped you feel more comfortable about [a goal of the class]?
Why would you recommend the class to a friend or family member?
It may also be helpful to find out more about the individual and ask what other community activities they are involved in, or what a typical day looks like for them.

You are welcome to share whatever amount of information feels comfortable for you and that you would like others to know about your story and journey to healthy living.

Option 1 - For a specific program:
Please start the video by stating your name, the region/coalition/county you represent, where you participate in healthy aging programs, and what program you would like to highlight.
Please try to keep videos between 1-2 minutes.
You are encouraged to have someone else film the video but selfie videos will be allowed for entry.

Photo quality is best when you use the camera on the back of the smartphone instead of the “selfie” camera.
Wipe off the lens to make sure there are no fingerprint smudges.
Fill the frame with the subject and get closer if you need to.
Be sure that the subject has light on their face. For example, sitting facing a window.
